Employer Services Paperwork and Forms
For employer-paid physicals and pre-employment screenings, please note that additional paperwork may be required depending on the specific services needed. This paperwork will be provided at check-in to ensure a smooth process.
Authorization Requirements:
An authorization from the employer is required to bill for services. Please bring this authorization form with you to your appointment, or confirm that your employer has sent it to us ahead of time.
